YOUR SUMMARY CARE RECORD Your summary care record is an electronic record of your healthcare history (and other relevant personal information) held on a national healthcare records database provided and facilitated by NHS England. This record may be shared with other healthcare professionals and additions to this record may also be made by relevant healthcare professionals and organisations involved in your direct healthcare. You may have the right to demand that this record is not shared with anyone who is not involved in the provision of your direct healthcare. If you wish to enquire further as to your rights in respect of not sharing information on this record then please contact our Data Protection Officer. 1. Access and Subject Access Requests You have the right to see what information we hold about you and to request a copy of this information. We have one month to reply to you and give you the information that you require. Online Access You may ask us if you wish to have online access to your medical record. However, there will be certain protocols that we have to follow in order to give you online access, including written consent and production of documents that prove your identity. Correction We want to make sure that your personal information is accurate and up to date. You may ask us to correct any information you think is inaccurate. Removal You have the right to ask for your information to be removed however, if we require this information to assist us in providing you with appropriate medical services and diagnosis for your healthcare, then removal may not be possible. 1. About us

We are an award winning practice that has recently been recognised as an “Outstanding Practice” by the Care Quality Commission, which is the highest award possible. The practice has also recently received a Royal College of General Practitioners Quality Practice Award for the second time running. We offer personal general practitioner and family medical services to patients in Little Stoke, Bradley Stoke, Patchway, Stoke Gifford, Charlton Hayes and Filton from a modern purpose built health centre which has been extended to include additional consulting rooms in 2013. In December 2017, a further extension was completed which provided our patients with a further 20 consulting rooms, a health promotion facility and other accommodation for additional services. Our Practice is in Little Stoke on the northern edge of the city, approximately 5 minutes from the M4/M5 junction. The practice population lives mainly in the Little Stoke, Bradley Stoke, Patchway, Stoke Gifford Filton and Charlton Hayes areas. The Practice is part of the Bristol, North Somerset and South Gloucestershire Clinical Commissioning Group. The area consists of a variety of housing mostly built within the last 20 years. A high proportion of the practice population consists of families with young children, (only 9.2% are aged over 65). There are small pockets of deprivation, mostly in the Patchway area. The main Practice was founded in the 1930’s in North Filton, and moved to Little Stoke in the ‘50’s. The Practice moved to it’s current premises in 1988. The surgery consists of 12 consulting rooms with 4 treatment rooms plus admin offices. There are currently 3 Partners, 8 Associate GPs, 1 Mental Health Nurse Consultant, 2 Musculoskeletal Consultants and 2 Clinical Pharmacists. We have 5 Practice Nurses, 3 Nursing Assistants and 2 Phlebotomists. The administration and management side of the Practice is supported by 19 members of staff including a Practice Manager. All GP Practices are required to declare the mean earnings (e.g average pay) for GPs working to deliver NHS Services to patients at each practice. The average pay for GPs working in Concord Medical Centre in the last financial year was £42,380 before tax and national insurance. This is based on 6 partners, 5 salaried GP’s and 3 locums (regular) who worked at the practice for more than 6 months.
